The following two (2) pages must be printed, signed, and submitted to our office staff at FOCUS Therapy. One is a member consent form for insurance payment processing, and the other is a release for healthcare information.
You can physically hand these forms into our office staff, or you can scan them as JPGs or PDFs and send them to us at [email protected], with the title, "Medical and Insurance Release Forms for (Your Child's Name)."
